The program is individually designed to help you take control of your diabetes and prevent long-term complications of the disease.
The outpatient diabetes education program is open to anyone with Type 1 or Type 2 diabetes, newly diagnosed or longstanding. Our educators will work with you, your family and your physician to create your personal diabetes care plan.
