A: For an official copy of your birth certificate, please click here for the City of Detroit website or here for the DMC website.
You may also go to the Detroit Health Department, Vital Records Department, Herman Keifer Health Complex, 1151 Taylor N., Detroit, MI. Hours: 8 a.m. - 4 p.m. Phone: 313-876-4049 or 313-876-4133. Cost is $17.

A: For medical records, click here.
You may also call 313-966-4166 to obtain a medical release form. Once completed, please mail to: DMC Sinai-Grace Hospital, Medical Records, 6071 W. Outer Drive, Detroit, MI 48235 or fax to 313-966-2850. You must be 18 years of age to request your medical records. Unless you are a parent or guardian, we can’t release anybody’s medical records to you but your own.
